Flexible Care Option™

The Three-cycle Gonadotropin/IUI Option™

Services that are included in this option are based upon the usual number of office visits, ultrasounds and blood work required during an average Gonadotropin/IUI Cycle. Clinical care is provided following the cycle until a serum pregnancy test is performed. Obstetrical and other medical services are not covered as part of this program. If a pregnancy loss occurs during the 12 month benefit period, additional infertility services are provided through the balance of the 12 month benefit period, up to the maximum of services. The cost of The Three-cycle Gonadotropin/IUI Option is established after your initial consultation with an ARC participating physician.

The predetermined fee for this program must be paid in advance OR through The ARC Affordable Payment Plan™. The fee is payable regardless of when or whether the Patient conceives during the program. The maximum of services must be completed within 12 months from the date of the initiation of cycle.
