How To Sign Up

1. Learn about The ARC Fertility Program.
     
   • Read about the program on the website  
       
   • Call ARC toll free at (888) 990-2727 for more information or to ask questions
       
2. Complete The ARC Fertility Program Application™.
   
   
3. Submit your Application to ARC. You may fax your completed application to ARC at (877) 655-2727. Once received by ARC, you will be sent The ARC Patient Participation Agreement listing each of The ARC Treatment Package Options™ and your prices for each. If the Application is complete, you will receive the Agreement form with prices within 3 working days of its receipt. If you have submitted a completed financial application, you will also know at that time if you have been approved for financing.
   
   
4. Make your selections.
   
   

   The ARC Treatment Packages™.  Decide which of The ARC Treatment Package™ you wish to purchase: The Comprehensive Initial Consultation™ One-cycle Plus™ Two-cycle Plus™ Three-cycle Plus™ The Flexible Care Option™ Comprehensive Care Option™ The ARC Affordable Payment Plan™. Decide if you want to use ARC financing to purchase your care. The ARC Refund Guarantee™. If you have chosen to purchase Three-cycle Plus™, you are also eligible to purchase The ARC Refund Guarantee™. This Plan provides a refund if you do not achieve a live birth during the treatment period or within the following 10 months. You may choose to purchase this Plan for reimbursement of some or all of your expenditures. The price and terms of this Plan are provided with your Patient Participation Agreement.

5. Sign and return your Patient Participation Agreement to ARC to fax number (877) 655-2727 and the original Agreement in the return envelope.
   
   
6. Initiate care.
   
   Schedule care with your physician. Talk with your physician, nurse or IVF coordinator about initiating care. Please let the practice know that you are purchasing care through The ARC Fertility Program. This will indicate to them the payment arrangements.

1. If you are paying for your care without financing, you will pay ARC by check.
2. If you have chosen financing, you will be making monthly payments directly through The ARC Affordable Payment Plan™.
3. It is not necessary for you to pay your physician anything unless you are provided services that are not included in your chosen ARC Treatment Package™.
4. In the Program, the only thing required of the practice is that they notify ARC that your care is continuing according to the Treatment Package you have purchased. This triggers payment to your physician by ARC.
