Final Steps
- ARC will send you a Patient Participation Agreement (PPA), for your signature, specifying the price of each ARC package.
- You will return the signed PPA with payment (personal check or Visa / MasterCard / American Express / Discover.

Once your financing or payment method has been determined and secured, contact us to purchase your clinical package.
As soon as your Patient Participation Agreement is returned to ARC, the loan documents will be sent to you for signature. As soon as those are returned, you can start you treatment.
- We will contact your physician's office and notify them of the clinical services you have chosen once you have completed Final Steps 1 and 2.
- ARC will pay the practice directly for the services you have purchased.
You will be responsible for preliminary lab work needed prior to the start of an IVF cycle and should refer to the Treatment Package Descriptions to determine what is included and excluded in clinical package you have purchased.
ARC recognizes that in some situations it may be in the patient's
best interest to cancel an ARC Treatment Package. The ARC Cancellation Policy
establishes the requirements and conditions to cancel an ARC Treatment Package
after it has been purchased.
